Should a player be given an error if she dives for a ball (when necessary) and ball is not caught

No but the extra ordinary effort may result in an error(throw). That is why fielding % is flawed. Better range = more difficult chances. Calling a dive ordinary effort is one thing, its another to complete the play.

Necessary for one is not necessary for another.

Yes there are times when player Dives and the ball goes in their glove and rolls out. Error

Better range = more difficult chances.
I don't believe this. Better range = more chances.

If you have to dive or back hand a hard hit ball that is 3 feet off; and I have worse range, and have to dive or backhand a ball 2 feet off; is just as difficult for me at 2 feet as you at 3 feet. I don't get the error on my dive because I was only able to take 2 step before diving.
