Batting out of order isn't automatically an out per the USA Softball rules. We had to look it up because it happened to my kids' team last season (girls got silly and stopped paying close attention). In fact more often than not it's not an out. It's only an out if it's discovered right after the wrong batter but before any pitch to the next batter. If it's in the middle of the wrong batter the right batter just assumes the at bat. If the next batter is already up, then you keep going after wherever you are lineup (so people get skipped, pay attention next time).
That is correct. Once a new batter receives their first pitch, actions before that are validated.

Young girls, Rec league, batting out of order.........nothingberger. Unless there is a homerun hitter that comes up to bat every time.
