As an umpire in TX, I play the game, then if the other team brings it up I'll (maybe) fill out a UIL Incident Report after the game. I'm certainly not going to not play the game or anything.

well said, but, as an official working the game that's not your call. If the coach requests time, and its within the rules of that game, then it should be granted ... IMO
as an umpire why would you care about this? would you rather end a game on time or go extra time? just enforce the rules ... its not illegal to call a time out with 2 minutes to go.
"hesitated even by the minutest amount" --- for most umps, "immediately" (what is in the rule) probably means 1 second minimum, and maybe as long as 2 seconds for the younger girls. The runner need to be allowed to round the base, stop, assess, then decide to go fwd or back.

You have no call until the def. team appeals. If they don't prior to the next pitch, then all is good ... runner at 2nd, play on. If they do, R2 is out, R1's run stands.
The right thing for the ump is to call/signal OBS every time they see it. 95% of the time no bases will be awarded, but it should be called 100% of the time - never ignore it.
