Like others have said, impossible to judge without being there and seeing what is going on. Other than reasons already stated, it could be a simple as "forcing" the 3rd baseman to play up.
I dont understand your question. The coach's only "discipline" should be lack of playing time. If a parent is doling out discipline for sports, they have mental issues.
The coaches see the players everyday. If they cant tell who is better a couple weeks into practice, they shouldn't be coaching. Pitchers are no different to evaluate than anyone else. Ive seen girls that throw harder and have more movement and then get hit all over the ballpark when an average...

Except the coaches didnt assume anything. They were directly told what the rules were going to be. The umpires even acknowledged that the way they were playing was different than the printed rules. This "mistake" is not the fault of the coach.
Im not an umpire so I cant say when a plate umpire should make a call, but in this case, it seems like the plate umpire would have a better look as to 1B coming off the bag. I dont know why she would ask for help when she is looking directly at the play?
